Job Description:
Position Summary:
The Home Health Aide within this organization will perform duties
necessary to assure the cleanliness and personal hygiene of the
patients assigned to them. All patients are treated with respect,
without discrimination. The personal care required by each patient
will be determined by the patient's primary care nurse and will be
documented on the Home Health Aide Assignment Sheet. The aide will
be directly supervised by the Clinical Manager, but will also
accept direction from the Clinical Supervisors and the patient's
primary nurse. Performance of such duties will be done in
compliance with all agency policies and standards of practice.
Evaluation of duties will be done monthly on each patient during
regular administrative visits. Home Health Aides are responsible to
remind forgetful patients to take their medications, which have
been set out for them by their nurse or family ahead of time. HOME
HEALTH AIDES MUST NOT DISPENSE MEDICATIONS.
Essential Functions and Responsibilities:
1. Review and follow home care plan developed by the primary nurse,
perform the assigned duties, and document care given according to
the policies and procedures of the organization. (Total personal
care of a patient includes: total or partial bed bath, tub bath or
shower, mouth care, denture care, shampoo and grooming of hair,
shaving, complete peri care, changing adult diapers, giving back
rubs and observe for changes in skin integrity.)
2. Take vital signs including: temperature, pulse, respirations and
blood pressure, as assigned, and know normal ranges of each. Report
all vital signs that are outside the normal range.
3. Care of patients with special skin problems includes: assessing
potential for breakdown, protecting pressure points, turning the
patient, applying treatment patches to broken areas after bathing,
cleaning of the area with prescribed solutions, and preventing
pressure sores by positioning of the patient.
A. Care of patients with infectious processes, i.e., generalized
wound and skin.
4. Care to provide a safe and clean environment includes: cleaning
the area around where the bath has been given to assure that floors
are not wet and the area is clean, cleaning the area around the
bed, making the bed or changing linens, and assuring that all
clutter has been removed from the areas that the patient walks
through. Always treating property with caution and respect
5. The aide must recognize and report any unusual, untoward, or new
symptoms/ changes to the RN.
6. Care for patients with urinary system deficits includes:
catheter care (either internal or external), measuring output and
assessing intake, and proper charting of I & O.

Required:
--- Graduate from an accredited high school program.
--- Successful completion of a written Home Health/Hospice
competency test and the clinical skills testing, as evidence of the
necessary skills for Home Health Aide employment. Test must be
passed before the end of Employee's probationary period.
--- State CNA Certification
--- Current Michigan driver's license, proof of insurance, a
reliable vehicle and be a responsible driver.
--- Favorable references from all work places.
Preferred:
--- Minimum 6-months to 1-year of experience with a variety of
patients..
