Job Description

General Purpose of Job:
As part of the Department of Population Health, the Community Dietitian will be part of a larger multidisciplinary team supporting Sparrow Medical Group (SMG) primary care patients currently enrolled in Chronic Care Management (CCM). As part of a grant-funded project, the employee would have the opportunity to be an integral part of a two-year start-up program in partnership with a local food pantry, where additional support will be provided to patients who screen positive for concerns related to food insecurity.
The position will be embedded within the local food pantry and receive referrals directly from the Registered Nurse Care Facilitators (RNCFs) assisting in the identification of a subset of the population served. The goal of the program will be to provide in-depth nutrition counseling and appropriate resources to assist in lifestyle modifications and self-management training. The candidate will be assisting in the planning and implementation of the program, which will require the ability to be flexible and innovate through an iterative change management process.
Essential Duties:
This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.


  • Acts as a liaison and coordinates the clinical requirements of the Michigan Health Endowment Fund (MHEF) grant for the Michigan Department of Health and Human Services (MDHHS).
  • Independently oversees the day-to-day operations of the program, acting as an extension or resource for SMG caregivers including physicians and support staff.
  • Tracks key performance metrics for the grant including patients served, basic biometrics, and observed outcomes.
  • Works in conjunction with the larger care coordination team to follow CCM patients, while providing nutritional assessments by phone or in-person.
  • Schedules patients in a timely manner to receive the needed care based off practice feedback, risk score, and/or initial screenings.
  • Documents all patient encounters within Epic, clearly identifying the nutritional care plan and adhering to basic EMR guidelines and follow-up requirements.
  • Assists in supplemental screening activities, identifying patients activated and ready to make change.
  • Creates and delivers nutrition education in creative ways, working with a diverse patient population.
  • Develops nutrition training for both individual and group classes, which may include example shopping lists, easy-to-implement recipes, and grocery store tips and tricks.
  • Facilitates cooking classes or demonstrations for active participants.
  • Counsels patients and family members on nutritional needs, diet modifications, and/or food and drug interactions.
  • Partners with additional Community Based Organizations (CBOs) where appropriate to provide additional resources.
  • Partners in quality improvement initiatives where needed.
  • Performs other duties as assigned.






    Job Requirements



    General Requirements
    *Current registration with the Commission on Dietetic Registration (CDR). *Valid driver's license. *Basic Life Support (CPR). *Certification as a Diabetic Educator (CDE) - preferred.


    Work Experience
    *Minimum two years of experience within an ambulatory clinic or community-based environment. *Experience participating in a grant-funded project.


    Education
    *Bachelor's degree in Dietetics from an ASCEND accredited college or university. *Completion of an ASCENT accredited dietetic internship. *Master's degree in nutrition or nutrition-related field - preferred.


    Specialized Knowledge and Skills
    * An understanding of care management/case management principles. * Knowledge of nutritional values of food and the therapeutic values of planned menus. * Knowledge of regulatory standards and health system policies related to assigned functions including HIPAA confidentiality, State of Michigan, ADA, and TJC mandates. * Written and computer skills necessary to prepare required documentation. * Interpersonal skills necessary to effectively communicate with patients, families, physicians, and health care professionals in the exchange of information. The incumbent is also required to effectively teach adult learners with varying learning abilities and understanding of the English language. * Analytical ability to assess patient's nutritional needs and develop a care plan based upon physician/practice instructions and patient food preferences. * Mental ability to adapt and respond to multiple priorities and demands and meet the instructional needs of patients and departmental staff. * Physical ability to transport instructional materials/equipment weighing up to 50 pounds. * Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job. * Clinical understanding of major chronic diseases - preferred.
