• Provide compassionate patient care by triaging phone calls, completing patient care visits, attending death visits, and ordering supplies and equipment.
• Lead and supervise weekend care teams, ensuring smooth operations and continuity of care.
• Conduct hospice assessments, symptom management visits, and support patient admissions.
• Triage patient and family phone calls, providing timely clinical judgment and guidance.
• Support and educate families, caregivers, and patients during critical end-of-life transitions.
• Collaborate with interdisciplinary teams to maintain consistent, compassionate care.
• Mentor nursing staff, promote team cohesion, and uphold care standards during Weekend hours.What We're Looking For:
  • Licensed RN in the State of Michigan (required)
  • 2+ years of nursing experience; hospice or acute care leadership preferred
  • EMR proficiency required
  • Strong communication, decision-making, and leadership skills
  • Comfort with independent field work and triaging
