Job Description Job Description Nurse Coordinator 1st Call Home Healthcare is hiring a full time RN/LPN Homecare Nurse Coordinator to cover our West Michigan Territory. Must love to drive coverage area includes Grand Rapids, Battle Creek and Kalamazoo areas. $$$ 5,000 Sign on Bonus $$$ The Nurse Coordinator assumes responsibility and accountability for the delivery of industry recognized home care nursing and health related services to the agency's patients Duties Include: Perform start of care visits and establish care plans. Monthly follow up visits.
