Job Description The Clinically Integrated Network (CIN) Care
Management team will work in collaboration with the client,
physician(s), and other resources to ensure adherence with plan of
care to achieve an optimal level of wellness for each client. The
CIN Care Manager will perform duties as a registered nurse, case
manager, and an educator to assess, coordinate, and educate the
high risk and rising risk client population to achieve their
optimal level of wellness. SCHEDULE & SHIFT Monday - Friday: 8:00
AM - 5:00 PM ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS Collect,
analysis, and report data with regard to the client population of
the CIN Care Management program. Assess plans, implements,
documents, coordinates, monitors, evaluates, and updates the plan
of care through collaboration with the client’s healthcare team.
Executes specific care management activities and/or interventions
that will lead to accomplishing the goals set forth in the plan of
care. Researches, evaluates and recommends resources to meet needs
of the client. Act as a liaison for the client between physicians,
outpatient and inpatient facilities, as well as home health
agencies. EDUCATION AND EXPERIENCE Must be a high school graduate
or equivalent. Current State of Texas License as a Registered
Nurse. Five years of nursing experience, Case Management experience
preferred. Ability to apply nursing theory, evidence-based nursing
knowledge, problem solving skills and therapeutic techniques to
perform professional nursing duties safely, legally, ethically,
efficiently, and effectively.
