Job Description

General Purpose of Job:
Provide pre-admission evaluations for patients referred for treatment to the inpatient rehabilitation program. Obtain appropriate pre-certifications with appropriate payers. Adheres to pre-admission guidelines prescribed by CMS, Interqual, Blue Cross and other insurance providers and managed care programs. Establishes referral bases with professionals within the Sparrow Health System and other external agencies.
Essential Duties:
This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.

  • Performs pre-admission screening of all patients referred to the inpatient rehabilitation program. This includes chart review, interview with patient / family and collaboration with current patient caregivers (i.e., RN, Physicians, Therapists etc.) with attention to the patients rehab potential, therapy progress, discharge plan and feasibility and special equipment needs.
  • Develops functional short and long-term goals to determine if the patient has met the admission guidelines established by the payer.
  • Performs pre-certifications of patients that have met the pre-admission guidelines communicating the rehab plan of care.
  • Provides recommendations as to the appropriate levels of care for patients not admitted to the inpatient rehabilitation program.
  • Maintains a thorough knowledge of funding and admission guidelines established by CMS, Interqual, third party insurance, no-fault auto and managed care organizations.
  • Communicates patient information to relevant members of the healthcare team.
  • Acts as liaison between members of the health care team, the patient / family, funders and external case managers.
  • Provides timely feedback / statistics on referral information including, but not limited to referral source, diagnosis, payer mix, patient demographics etc. for program development.
  • Performs tours of the inpatient rehabilitation facilities to patients / families and potential referral sources.
  • Develops and maintains relationships with referral sources.
  • Conducts focus groups with Stakeholders and implements corrective action when appropriate.

    Job Requirements

    General Requirements
    * Current Michigan RN Licensure and CRRN Exam eligible (Certified Rehabilitation Registered Nurse - Certified through Association of Rehabilitation Nurses), or * State of Michigan PT licensure, or * State of Michigan OT licensure and registered as an Occupational Therapist by the National Board for Certification in Occupational Therapy Inc, or * State of Michigan Speech-Language Pathology licensure and Certificate of Clinical Competency in Speech/Language Pathology from the American Speech-Language-Hearing Association, or * State of Michigan COTA licensure, or * State of Michigan PTA licensure, or * State of Michigan RT licensure

    Work Experience
    * None

    * Graduate of an accredited Associate Degree in Nursing and/or currently enrolled in BSN program or * Graduate of an accredited school of Physical Therapy or * Graduate of an accredited school of Occupational Therapy or * Master's Degree in Speech/Language Pathology from an accredited program in Speech/Language Pathology or * Graduate of a Respiratory Therapy, Physical Therapist Assistant or Occupational Therapy Assistant program

    Specialized Knowledge and Skills
    * Familiarity with resources available for use in assessing regulations and the parameters of third party reimbursement. * Excellent patient assessment skills and the ability to determine the appropriate level of rehab services. * Demonstrated expertise in decision making, time management, oral and written communication, and computer use. * Ability to function independently. * Analytical skills necessary to assess trends and data for program development. * Ability to provide informal and formal education to patients, community organizations, payers and other members of the health care team. * Comfortable presenting in front of groups of 1-50 people. * Intermediate expertise with development of PowerPoint presentations. * Intermediate expertise with use of Excel documentation. * Experience creating and editing marketing materials. * Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job.
