Job Description:
Trinity Health At Home is seeking a dedicated Hospice Case
Manager to join our compassionate team on weekends. This role is
pivotal in providing holistic, home-based care to patients with
life-limiting illnesses, ensuring they maintain dignity and comfort
in their final stages. The Case Manager will coordinate and provide
hospice services, support family members, and enforce personalized
care plans. This position requires strong interpersonal skills,
deep understanding of palliative care, and the ability to make
critical decisions compassionately and efficiently. Assess patient
needs and develop individualized care plans in collaboration with
physicians, patients, and their families. Manage a caseload of
hospice patients, providing regular visits and direct care on
weekends. Educate families and caregivers on patient care and
comfort measures. Coordinate with the interdisciplinary healthcare
team to ensure thorough and continuous care. Document patient
status and care provided with accuracy and timeliness in the
patient's clinical record. Manage resources efficiently to ensure
patient needs are met in a timely and effective manner. Participate
in on-call duties as necessary. Strong clinical skills in
palliative and end-of-life care. Effective communication and
interpersonal skills. Ability to work independently and as part of
a team. High level of empathy and commitment to patient dignity.
Proficient in documentation and patient management software.

401k Current Registered Nurse (RN) license in the state of
practice. Certified Hospice and Palliative Nurse (CHPN) is
preferred.
